It is imperative that we have current and valid telephone numbers to reach you in an emergency situation. Please keep us updated throughout the school year of any changes on Genesis Parent

Access:  

1. Under Forms tab - Complete the Health Information form for each child. 

2. Under Contacts (button in upper right corner)      

3. Review  and update your phone and email information (Guardian 1 and Guardian 2)    

4. Provide at least two emergency Contacts with phone numbers. The Emergency Contact should be a neighbor or nearby relative who will assume temporary care of your child if you cannot be reached. The Emergency Contact SHOULD NOT be your child's doctor.

Note: Once completed, the Health Information and Contacts will be available for viewing by the school nurse.
